Educational questionnaire
The GLP-1 questionnaire keeps your answers in the open page’s memory. Answers clear when you reload the page or choose “Clear my answers.” Height, weight, BMI, conditions, medication history, safety answers, and your result are not sent to MailerLite, booking links, or advertising analytics by the questionnaire. Results are educational and do not determine eligibility for treatment.
Guide requests & education emails
Request the guide by providing your email and guide-delivery consent. MailerLite processes these to provide access and email the requested resource. A guide request does not enroll you in ongoing marketing. Any education subscription requires separate consent, and each email includes an unsubscribe link. Please do not provide medical details in signup forms or ordinary email.
Website services
Framer hosts this website. MailerLite hosts email signup forms and the guide file. These providers may process technical information such as IP addresses, browser details, and service cookies. The site also includes Google Tag Manager; any website measurement must be configured to exclude medical responses and contact information. Questionnaire answer collection and advertising measurement inside the assessment are disabled.
Booking & clinical communication
Consultation booking opens Healthie, a separate service with its own privacy practices. Use the practice’s secure clinical communication channel for personal medical information. This website notice covers the website and email resource flow; clinical records and patient privacy notices are handled separately during care.
Your choices
You may use the questionnaire without joining an email list. Guide requests do not enroll you in ongoing marketing. Use “Clear my answers” to reset the questionnaire, and the unsubscribe link in any email to stop those messages.
